The concept of a sixth sense, or the ability of normal people to perceive the supernatural, is one of the foundational ideas of the Fatal Frame series, and many characters in the series possess this trait. Although a sixth sense is not necessary to see ghosts, it does determine a character's strength in battling them, as the Camera Obscura is more powerful in the hands of someone with a strong psychic ability.

A character's sixth sense can manifest in many different ways. Most commonly, the character will simply feel a sense of foreboding about a place or situation. They will be able to sense the presence of invisible ghosts and receive impressions, prophetic dreams or even visions concerning a place's history, although they will not necessarily understand what they see, and can even be driven mad by the horrors they have to witness. Characters with a strong sixth sense are also more susceptible to the vengeful emotions of the ghosts, and under certain circumstances may fall under the influence of a very powerful ghost and be forced to play a role in the re-enactment of a disaster.

In the Fatal Frame series, a sixth sense is generally an innate ability, with different people having naturally different levels of sensitivity, though the ability to see spirits can also be an effect of Overexposure. The sixth sense seems to run in families, passing from parents to children. Sometimes siblings share the ability, and will be more sensitive together than apart. The games also suggest a strong psychic bond between twins, allowing them to feel each other's pain and know what is happening to the other when apart. According to Japanese folklore, and reflected in the games, women are more likely than men to possess a powerful sixth sense.

It is possible for psychic ability to disappear or become dormant, as Miku Hinasaki's does for a time after the death of her brother, Mafuyu.

Much of occultist Dr. Kunihiko Asou's technology has to do with making the phenomena of the spirit world accessible to people without a sixth sense; for example, the Spirit Stone Radio transmits the thoughts of the dead trapped in crystals, and the Camera Obscura and Projector reveal things that could not be seen using normal apparatus.
